  • Annual Tax and Wage Report For Domestic Employment - DOL 4A (PDF, 225.15 KB)
    Annual tax and wage report which domestic employers must file. The annual report and any payment due must be filed on or before January 31st of the following year to be considered timely.This form is interactive and can be completed electronically and printed.     New employers with private, agriculture, and domestic employment can submit an Online Employer Tax Registration (https://www.dol.state.ga.us/Access/Service/LogonDOL1) and obtain a GDOL account number, if liable, upon submission.  Paper applications can take up to 30 days for processing and the determination is mailed to the address of record. Non-profit organizations, governmental agencies, businesses that change their ownership structure, businesses that merge, and businesses that acquire assets from other businesses must continue to use the paper application. This form is interactive and can be completed electronically and printed. The warning 'This form is not supported with the current version of Acrobat or Adobe Reader' is because Adobe Acrobat Reader is not set as the default PDF viewer for your browser. Change the browser default PDF viewer (https://helpx.adobe.com/acrobat/using/display-pdf-in-browser.html).

    This authorization form may be used by individuals to request the release of their unemployment records or information to a government agency or non-profit organization for the purpose of obtaining food, medical, financial, or housing assistance. An individual may also use this form to request the release of their unemployment records to a third-party providing assistance with their unemployment claim or aid in obtaining the aforementioned types of assistance. This authorization form is not approved for use by parties to judicial or formal administrative proceedings. Please contact the Legal Office at 404-232-3310 should you have any questions.
