This authorization form may be used by individuals to request the release of their unemployment records or information to a government agency or non-profit organization for the purpose of obtaining food, medical, financial, or housing assistance. An individual may also use this form to request the release of their unemployment records to a third-party providing assistance with their unemployment claim or aid in obtaining the aforementioned types of assistance.
