Power of Attorney Information

You may elect to appoint an agent to represent your business in all matters affecting your unemployment insurance (UI) taxes with or without limitations related to UI claims. To assign a representative, complete the applicable Georgia Department of Labor (GDOL) Power of Attorney form. The completed form can be submitted to GDOL by email, fax, or regular mail to:

Please note: GDOL does not accept power of attorney forms from other state or federal agencies, such as the Internal Revenue Service (IRS) or Georgia Department of Revenue (DOR).
